Primary Partners Alliance Demonstrates Nearly $70 Lower PMPM Healthcare Costs Using Lightbeam Health Solutions

Analysis highlights lower costs and utilization, fewer readmissions and higher HCC recapture through an integrated population health strategy

DALLAS, Oct. 7, 2026 /PRNewswire/ — Lightbeam Health Solutions, a proven AI-enabled solutions and services leader helping healthcare organizations succeed in value-based care, today announced results from Primary Partners Alliance (PPA) population health strategy, which leverages Lightbeam’s platform to identify patient risk and care gaps, prioritize outreach, and support more coordinated care.

A one-year analysis of PPA’s Medicare Shared Savings Program (MSSP) and ACO REACH population found that patients that PPA successfully identified, engaged, and supported through a completion of an Annual Wellness Visit (AWV) consistently demonstrated better outcomes than those who did not complete an AWV. The findings reinforce PPA’s approach of using AWVs as an entry point to ongoing patient engagement and care management.
