IBM Study: Eight in Ten EMEA Enterprises Escalate AI Incidents to Board or C-Suite as AI Agents Test Governance

Published

on

80% of surveyed executives in EMEA say an AI-related incident reached their board or C-suite in the past year, with only 40% changing AI governance policy or frameworks in response.75% of respondents expect AI agents to have some level of autonomy with respect to decisions by 2027, yet only 37% say their governance is designed to scale with agentic AI deployment. When surveyed organizations want to deploy AI faster than governance and compliance frameworks are prepared for, nearly 60 percent (56%) continue within agreed risk limits while governance adapts; a third (33%) delay deployment until governance is ready.Only 37% of respondents have a formal, board-reviewed quantification of the financial risks created by gaps in their AI governance.

LONDON, Oct. 15, 2026 /PRNewswire/ — New research by IBM (NYSE: IBM) finds that AI-related incidents are reaching boardrooms across Europe and the Middle East among the organizations surveyed, but governance capabilities and risk accountability are not keeping pace with adoption of the technology.

Eight in ten executives surveyed say an AI-related incident – including an agent failure, data exposure, AI-driven decision error, or AI security breach – reached their board or C-suite in the past year. Yet fewer than half (40%) of those organizations cite a change to AI governance policy or framework in response.

When the pace of deploying AI outstrips governance and compliance frameworks readiness, rather than pause while governance adapts, the majority (56%) of executives surveyed report pressing ahead within agreed risk limits. Just a third delay deployment entirely until their governance is ready.

The survey, of more than 3,200 senior executives at organizations across eight countries in Europe and the Middle East, suggests governance policies will be tested further as AI agents take on more autonomous work. Thirty-five percent of respondents say they are running agentic AI in production today, yet three-quarters expect AI agents to have some level of autonomy towards decisions by 2027. Only 37% say their current governance system is designed to scale with agentic AI deployment.

Ana Paula Assis, IBM Senior Vice President and Chair, EMEA and APAC, said: “The AI debate has moved from adoption to governance and accountability. As organizations progress from AI assistants to autonomous agents, governance can no longer be treated as a compliance exercise. It has become a business requirement and value enabler.

The enterprises that can scale AI with accountability and trust will be postitioned to realize business value. That means governance must be built into how agents operate, with clear boundaries on what they can do, clear ownership when things go wrong, and controls that work in real-time.”

Accountability & liability gap

Nearly all executives (98%) surveyed have formally assigned responsibility for AI risk at board level, including 26% with a dedicated board-level AI committee. Yet the research found no clear consensus on accountability for adverse outcomes from AI agents. An organization’s CIO or CTO was the most frequently cited role with assigned responsibility for adverse outcomes from AI agents, selected by just 17% of all respondents.

Respondents’ views on legal liability when an AI system causes significant harm were also divided: 20% say it should be shared proportionally between deployer and vendor, 19% say there should be a regulatory framework that does not yet adequately exist, and 14% say it should sit with the AI vendor or technology provider.

To manage increasingly autonomous AI agents making decisions without human approval at each step by 2027, executives expect to need formal legal or contractual frameworks defining AI vendor liability for agent failures (33%), defined human escalation thresholds (32%), real-time monitoring and audit trails of agent decisions and actions (30%) and the ability to immediately halt agent activity across the enterprise (29%).

Other key findings include:

Measurement of financial risk exposure less established than board ownership: More than half (54%) of surveyed executives govern generative AI tools and agentic AI systems as distinct risk categories with separate governance frameworks and a further 38% apply the same core framework, but with additional controls to agentic AI. Measuring financial exposure from gaps in AI governance is less advanced: only 37% have a formal, board-reviewed quantification of the financial risk created by gaps in their AI governance.Supply chain risk: 54% of all respondents acknowledge gaps or limitations in how their governance framework covers the AI systems and models used by key vendors, technology partners and supply chain participants. Only 21% continuously monitor third-party AI risk and 13% apply independent assurance or certification requirements.Regulation shapes the pace regionally: In the EU, 41% of surveyed executives are accelerating governance investment ahead of the EU AI Act’s high-risk obligations in December 2027, and a further 35% are on track with structured compliance plans. In the UK, sector regulator expectations are the most cited driver of AI governance investment (27%), while in Saudi Arabia and the UAE, national AI strategy requirements lead (61%).

Study methodology

The research was conducted by Censuswide on behalf of IBM among 3,221 executive management respondents, including C-level, at organizations with 1,000 or more employees in the UK, France, Germany, Italy, Spain, the Netherlands, Saudi Arabia and the UAE. The data was collected between 14 and 24 September 2026.

World’s Largest Flying Digital Screen Makes NYC Debut with Two Flights and New Byron Allen Media Partnership

HELI-D flew over the Hudson River near Chelsea Piers and the Statue of Liberty during the DPAA Global Video Everywhere Summit, followed by an evening flight showcasing Byron Allen’s media content

NEW YORK, Oct. 14, 2026 /PRNewswire/ — HELI-D, the company behind the world’s largest flying digital screen, made its New York City debut on October 13 with two flights introducing a new form of aerial digital advertising to one of the world’s largest media markets. The debut featured an afternoon flight in partnership with the Digital Place Based Advertising Association (DPAA) during its annual Global Video Everywhere Summit, followed by an evening flight showcasing content from media mogul Byron Allen’s Allen Media Group as part of a newly secured partnership.

The afternoon flight took place between 1:00 PM and 1:45 PM ET, coinciding with the Summit’s lunch break. Launching from Newport Helistop in New Jersey, HELI-D flew along the Hudson River toward Chelsea Piers, where Summit attendees and New Yorkers had an opportunity to see the massive digital screen in action. The aircraft then headed south toward the Statue of Liberty, bringing the display to one of the city’s most recognizable landmarks.

HELI-D returned to the skies later that evening for a second flight featuring content from Byron Allen’s Allen Media Group. The new agreement marks a significant step in HELI-D’s entry into the U.S. advertising market, bringing together its aerial digital display technology and one of the country’s prominent media entrepreneurs.

Unlike traditional billboards fixed to buildings or roadside structures, HELI-D’s technology takes digital out-of-home advertising into the sky, creating a mobile, large-scale display capable of bringing brand messages directly to high-visibility locations, major events and large audiences.

“New York City is one of the most important advertising markets in the world, and there’s no better place to demonstrate what’s possible when digital advertising takes to the skies,” said Simon Powell, Co-Founder and CEO of HELI-D. “Launching with DPAA and The Ad Council, and now working with Byron Allen and his media company, makes this an incredibly exciting debut for us. These partnerships demonstrate the potential of our technology to bring major brands and media content to audiences in an entirely new way.”

The New York debut coincided with the DPAA Global Video Everywhere Summit, which brought together leaders from across the advertising, media and digital out-of-home industries. As a new media partner of DPAA and The Ad Council, HELI-D featured content from both organizations during the afternoon flight, demonstrating how its aerial display can be used for commercial advertising, public awareness campaigns and large-scale brand activations.

With two flights marking its New York City debut and new partnerships spanning the advertising and media industries, HELI-D is positioning its technology as a new addition to the digital out-of-home advertising landscape, offering brands the ability to deliver dynamic content above some of the world’s most recognizable destinations and major events.

Eight in Ten UK Businesses Struggle to Keep Pace with AI Governance, IBM Study Finds

LONDON, Oct. 15, 2026 /PRNewswire/ — New research by IBM (NYSE: IBM) finds that AI-related incidents are widespread across the UK, but governance capabilities and risk accountability are not keeping pace with adoption of the technology.

Eight in ten UK executives surveyed say an AI-related incident – including an agent failure, data exposure, AI-driven decision error, or AI security breach – reached their board or C-suite in the past year. Yet fewer than half (35%) of those organisations cite a change to AI governance policy or frame works in response.

The findings come as the UK Government advances its AI Opportunities Action Plan, which seeks to accelerate AI adoption and economic growth while enabling safe and trusted development through regulation, safety and assurance. The research suggests UK ambition is strong, but governance must mature alongside adoption if organisations are to scale AI with confidence.

When AI deployment moves faster than governance readiness, the majority (61%) of these organisations report pressing ahead within agreed risk limits rather than pause. The research indicates UK organisations are less hesitant than counterparts in EMEA – only a quarter (27%) of UK respondents report delaying deployment entirely until their governance controls are ready, compared to a third (33%) across EMEA.

The findings highlight a growing governance challenge as AI agents take on more autonomous work. UK respondents are among the most ambitious in EMEA when it comes to AI adoption: 68% expect AI agents to have some level of autonomy decisions by 2027. Yet only 53% believe their governance structures can scale to manage that future, highlighting a potential readiness gap.

Leon Butler, Chief Executive, IBM UK and Ireland, said:

“AI is moving faster than governance, and that gap is becoming a business risk.”

“With the increase of AI agents this changes the equation again. When technology can act independently, organisations need more than approval processes. They need governance built into the system itself becoming an enabler, not a constraint. The winners in the AI era will be those that can trust, govern and scale it with confidence.”

Julian David OBE, CEO of techUK, said:

“As AI becomes more deeply embedded in business operations, trust will be a key differentiator. Organisations need governance frameworks that enable them to innovate with confidence, ensuring AI agents operate transparently, responsibly and within clearly defined boundaries. The businesses that get this balance right will be best positioned to unlock AI’s full potential.”

Accountability & liability gap

Nearly all UK executives (97%) surveyed have formally assigned responsibility for AI risk at board level, including 31% with a dedicated board-level AI committee. Yet the research found no clear consensus on accountability for adverse outcomes from AI agents. An organisation’s CIO or CTO was the most frequently cited role, selected by just one in five (20%) of all respondents.

There is also no consensus on legal liability when an AI system causes significant harm: 20% say it should be shared proportionally between deployer and vendor, 16% say there should be a regulatory framework that does not yet adequately exist, and 15% say it should sit with the AI vendor or technology provider.

To manage increasingly autonomous AI agents making decisions without human approval at each step by 2027, UK executives expect to need formal legal or contractual frameworks defining AI vendor liability for agent failures (32%), defined human escalation thresholds (37%), real-time monitoring and audit trails of agent decisions and actions (33%) and the ability to immediately halt agent activity across the enterprise (27%).

Other key findings include:

Measurement of financial risk exposure less established than board ownership: More than half (64%) of surveyed UK executives govern generative AI tools and agentic AI systems as distinct risk categories with separate governance frameworks and a further 29% apply the same core frameworks but with additional controls to agentic AI. However, only half (50%) have a formal, board-reviewed quantification of the financial risk created by gaps in their AI governance.Supply chain risk: 46% of all UK respondents acknowledge gaps or limitations in how their governance framework covers the AI systems and models used by key vendors, technology partners and supply chain participants. Only 19% continuously monitor third-party AI risk and 14% apply independent assurance or certification requirements.

This included 504 respondents in the UK. The research was conducted between 14 and 24 September 2026.
